This form should be used for making special requests regarding STCS Tuition or Fees payment schedules.  If you need assistance in updating your bank information, please call FACTS at 866-441-4637, as we are unable to do that for you.

This form is NOT intended to be used to request withdrawal of your child from St. Theresa.  Please contact Susie Thompson at sthompson@st-theresa.org regarding withdrawals.

Special requests will be considered on a case by case basis.  Please know we will do our best to consider all requests, but cannot guarantee approval. Special requests are additionally only applicable for a term period (i.e., a school semester or school year, etc.) Please note: any time-sensitive special requests (e.g. changing bank auto draft date) should be made no later than one week prior to the auto draft to allow for processing.

The following are examples of a special request:

"I would like to change my tuition payment plan from the 10-month plan to the 11-month plan."  

"Please move my payment due date from the 21st to the 30th."
