Please complete the lunch option form if your volunteer needs lunch provided and will not be bringing their own lunch.

Which weeks will your student volunteer require the lunch option?  (Please note this is an additional charge of $30 per week.)  Mondays: Papa John’s cheese pizza with a side of apple sauce or fruit cup and dessert Tuesdays: Barberito's cheese quesadillas and chips & salsa from and dessert Wednesdays: Chick-Fil-A 6-piece chicken nuggets or (Mac & Cheese for vegetarians) with a side of chips and dessert Thursdays: Elizabeth’s Pizza buttered pasta with a side of apple sauce or fruit cup and dessert Fridays:  Harris Teeter Ham & Cheese Sub (Cheese Subs for vegetarians) with a side of chips and dessert *

Is your student volunteer is a vegetarian (or would prefer the vegetarian options each week)? (Wednesday Chick-Fil-A nuggets will be replaced with Chick-Fil-A Mac and Cheese; Friday Ham Subs from Harris Teeter will be replaced with a cheese sub from Harris Teeter.) *
