BLS Crew Rower Registration - Fall 2022
Welcome to Fall 2022 BLS Crew rower registration! A parent or guardian (not the BLS student/rower) must complete this registration form. Please read the following information and complete all pages. Questions with asterisks are required. You must click the "send" button to complete the registration and save your answers! 

*IMPORTANT* Please list the email address for all parents/guardians who should receive correspondence about your rower. The team's listserv is composed using information provided on this registration form. Anyone not on the listserv may miss important information about the team/season.

Answers to many questions about the registration process, the Fall 2022 season and the BLS Crew team can be found at www.blscrew.org. Novice and Varsity practice schedules and program information are available at www.blscrew.org under the "Fall 2022" tab.

Because the Fall season is short, the team cannot accept new Novice rowers for the Fall 2022 season UNLESS they have participated in at least one week (and preferably two weeks) of a Learn-to-Row program. Students are encouraged to participate in BLS Crew's Summer Learn-to-Row Program if possible. However, if the dates do not work, other dates are available through Community Rowing, Inc. (CRI).

All Varsity rowers must submit their forms by August 26, 2022 and Novice rowers by September 9, 2022 although rowers are strongly encouraged to get the forms in place before school ends in June 2022. The rowers will need to upload the completed forms themselves to the BLS Crew Forms Submission Portal (https://forms.gle/9vTMjmMa69M8xq9S9). Forms can easily be scanned as PDFs using a free smartphone scanner app for this purpose ("Tiny Scanner" works well). You will also receive the link to the Forms Submission Portal in the email you receive confirming your rower's registration.

The required forms for all Fall 2022 rowers:

(1) BLS Sports Clearance: We strongly recommend that all rowers get their Fall Sports Clearance from the BLS Nurses' Office before the end of the school in June. The BLS nurses also make Sports Clearance appointments available in mid-August, but then will not provide additional appointments until after the start of school, so please do not delay as you will not be able to start practice if there are any difficulties or additional forms needed. To obtain a Sports Clearance from the BLS Nurses, you must provide (or have on file) a physician's school/camp form dated after 8/01/2021.

(2) Concussion Training Certificate: All rowers must complete the concussion training once per academic year. Instructions are here: https://www.blscrew.org/concussion-training.

(3) Safety Pledge: Download form here: https://www.blscrew.org/safety-pledge. Please sign and return a digital copy (print and sign, scan to submit Or edit PDF to sign and upload to submit).

(4) Opiate/ Substance Abuse Education: All student-athletes and parents must read the information regarding opiate and substance abuse provided here - https://www.bls.org/apps/pages/index.jsp?uREC_ID=203689&type=d&termREC_ID=&pREC_ID=405715 labeled "Opiate Substance Information". Additional information is provided here as well. You will be required to acknowledge that you have read the information prior to registering.

(5) Swim Test: New Novice rowers must submit a swim test form. Until a swim test is on record, the rower will be required to wear a life vest during practice on the water. The form and instructions on where swim tests are administered are here: https://www.blscrew.org/swim-test-form.

Rower fees for Varsity rowers are due by August 26, 2022 and for Novice rowers by September 16, 2022 UNLESS a financial aid or alternative payment arrangement has been made. BLS Crew is committed to ensuring that crew is accessible to all BLS families, and financial aid is available for families for whom the full rower fees are not affordable.
