We anticipate vaccines will be available beginning in late September and into October 2020. We will confirm receipt of this registration form and provide you with additional details within the next 5-7 days. At that time we will email you a Flu Consent Form. A consent form must be completed for each family member and submitted prior to your appointment at the drive-through clinic.
