CV Youth Baseball/Softball/T-ball Signup
Please make sure you fill out this form for each child.
Forms are due March 1st. 

The girls will play one doubleheader per week on Tuesday or Thursday. 

The boys' games will be played on Mondays and Wednesdays. 

T-ball will play in league, but dates will be determined later. 

Games will start in mid-May and run through the beginning of July with league tournaments to end the season. 
Both softball and baseball leagues split games between towns, trying to schedule teams at the same location if multiple teams are playing. They also try to have the same amount of home and away games for each team.

The cost for team membership is $30. This money goes toward equipment, league fees, insurance, and umpire fees. Each additional child in the same household will be $25 each. Payments can be made via Venmo or by mailing a check made out to Central Valley Youth Sports to P.O. Box 72, Greeley, NE 68842

I hereby waive and release any and all rights and claims this enrollee may have against the Central Valley Youth Sports Association or its representatives, coaches, officials, volunteers and sponsors for any and all injuries the participant may suffer in connection with participation in the event. I permit my child to participate in all activities. Players should provide their own personal equipment, water bottles, etc.  
I have read the above liability statement, and I understand that substantial rights are given up by signing this (marking Yes) release and do so voluntarily.

I hereby consent to my child's pictures and videos being taken on behalf of the Central Valley Youth Sports Association. I also grant the right to edit, use and reuse said products for non-profit, non-commercial purposes, including in print, online, social media and all other forms of media. I consent to the use of my child's name and association with CVYSA for the foregoing purposes. I give this authorization without expectation of compensation. 
This consent will remain in effect until I revoke it in writing.
