2021-22 Buckeye Local Schools Transportation Request/Change Form
(DO NOT COMPLETE IF YOU ARE TRANSPORTING YOUR CHILD TO AND FROM SCHOOL EACH DAY)      

At least a five to seven day notice must be given for requests/changes. Emergency requests will be handled with a bus pass issued by the building principal. Requests must be for five days per week to the same address. Day to day changes will not be permitted. Routes will not be changed to accommodate requests. Requests will be handled in the order received.

Deadline for transportation requests to be submitted for the 21-22 school year is July 21, 2021.  Requests received after this date will not be processed until 1 week after school starts.  Parents will be responsible for student transportation until then.
