Health and Travel Declaration - Workshop

Hello! Your safety is our priority and as part of our studio's precautionary measures to reduce the potential exposure of COVID-19 for our community and visitors, we would be conducting a simple temperature screening & declaration before each session.

If you do not satisfy any of the statements below, we would unfortunately not be able to conduct the workshop for you at present, but we would be happy to prioritise your booking for another date.

1. HEALTH *

I do not have the following symptoms today (even if mild): Fever / sore throat / cough / runny nose / loss of taste or smell etc.

I am not currently on MC (Medical Certificate) for respiratory symptoms.

2. TRAVEL *

I have not been outside of Singapore in the past 14 days.

3. PRECAUTIONARY MEASURES *

Neither I, nor my family members or housemates, are currently placed under Home Quarantine Order (QO), Leave of Absence (LOA) or Stay-Home Notice (SHN) due to confirmed or suspected COVID-19 cases and/or overseas travel to Ministry of Health's designated areas. I have not come into close contact with someone who is a confirmed COVID-19 case; or is part of a COVID-19 cluster in the past 14 days.

I have not visited any foreign worker dormitories in the past 14 days.
