The following form is where you will list any and all extracurricular activities that you have been a part of while at Ripley High School.  (No outside organizations or activities).  This will include clubs, sports teams, band, theater,  honorary organizations (Rho Kappa, National Honor Society, etc.), etc.  Classes/programs you have taken are not listed here.   Please also tell me what grade(s) you were in while participating in those activities.  
The last two questions are related to your post high school plans so I can forward you any information that would come through the counseling office.
