Please read the following information carefully and complete the booking form.

This booking form is to attend the ASIST workshop: Applied Suicide Intervention Skills Training.

This is a face to face workshop. Training is an un-restricted activity during lockdown and the tiered systems, however please rest assure that all necessary precautions re covid-19 will be taken to ensure your safety over the two days. More information will be sent via email regarding this.

ASIST is an active learning experience with regular opportunities to put learning into practice.

Over the course of the two-day workshop, ASIST participants learn to:
- Understand the ways that personal and societal attitudes affect views on suicide and interventions
- Provide guidance and suicide first aid to a person at risk in ways that meet their individual safety needs
- Identify the key elements of an effective suicide safety plan and the actions required to implement it
- Appreciate the value of improving and integrating suicide prevention resources in the community at large
- Recognize other important aspects of suicide prevention including life-promotion and self- care

This two-day workshop takes place over 15 hours, from 9am - 5pm. Participants must attend for the entire duration of the workshop.

Please do not attend in uniform.

Participants will be required to complete an evaluation form immediately following the workshop, and an electronic evaluation 3 and 6 months following course delivery.
