This Covid-19 Form is meant to notify NLL of a positive test result or Covid exposure of a NLL player or coach.

Please also notify your Division Coach. We will not reveal any names. This form will be sent only to Shannon Petrick, President, and Trey Christensen, Safety Officer.

Per our protocols:

If a player tests positive for Covid 19, or has confirmed exposure to someone who is positive, the player will be required to quarantine and not be allowed to rejoin their team until at least one of the following is true:

1) The player tests negative for Covid 19 at least 10 calendar days from when they tested positive. If they were exposed and did not test positive they will need to sit out 5 calendar days from the confirmed exposure then test negative after 5 calendar days; or
2) The player has been cleared by their physician to play and is proven to be not contagious or
3) The player is allowed to return to in person school.
