Dear parents,

If you want to keep the SAME classes, day and time, please just check appropriate buttons.
If you need to make CHANGES, please mark the day/time frame that works for you. Call or email if needed to discuss your new schedule.
With NEW request, please contact the office.
Please use one form for all children and all Art-Chess-Math-Ensemble Singing - Russian-Theater lessons (group or private); a different form for piano and other instruments and voice individual  lessons.

Please understand that your time slot (even if the schedule is confirmed) is only actually CONFIRMED when payment is submitted. If not paid within 48 hours, the time slot is considered open and can be offered to the next student who requested this time slot.

Terms and Agreements
I will be charged  Annual registration fee at the time of registration, and Monthly tuition fee on the 1st of each month starting on 9/1/2019 and ending on 6/1/2020.

Minimal commitment is 2 months

For withdrawal, I need to notify the office one month ahead

There are no withdrawals in May and June

Make-up lessons missed by a student are on teacher's discretion

There are no adjustments for lessons missed by a student

I will not approach Do-Re-Mi School teachers with any requests for out-of-school  engagements

I understand that supervising my children before, after and between the lessons is my responsibility
