2020 - 2021

This form contains the Activities Waiver and the Photo/Video/Audio Release. We need these two forms for each family so that students can participate in PTA sponsored events and have their photos in the yearbook as well as other LFES PTA media.

There is also a section to input your family information for use in the Online School Directory. If you choose to opt out of being published in the Online School Directory your information with be used only for LFES PTA communication.
