I grant permission to 318 Ministry, Celebration Church, Blountville, TN, its staff, employees, volunteers or representatives to take and use photographs and/or video footage of myself (participant) for varying purposes, including, but not limited to, social media/website posts, handouts, ministry settings, etc. 

I, the undersigned, am aware that risks are involved while participating in 318 Ministry activities/events, and hereby release 318 Ministry, Celebration Church, pastors, adult leaders, and 318 staff/volunteers from all liability. These liabilities include, but are not limited to, injury, loss of property or damage to property that may occur during the time 318 Ministry events are in session. If this student is injured and requires the attention of a medical professional, I give consent to any reasonable medical treatment as deemed necessary by the medical professional. In the event that treatment is necessary, I agree to hold any such person free and harmless of any claims, demands, and suits for damages arising from the giving of such consent. I also acknowledge that I will be ultimately responsible for the cost of any medical treatment should the cost of medical care not be reimbursed by a medical insurance provider. 

1. The purpose of this tournament is to fellowship + have fun !! We want to honor the Lord in all that we do and say :) and of course, get to play some pickleball !!
