I expect all athletes and coaching staff to observe social distancing and personal protective equipment (PPE) at all times during practice and competition. *
We have the ability to check temperature accurately at home, and will do so before in-person athletic activities. *
My son/daughter has access to PPE and understands the importance of wearing it when possible. *
I understand the importance of keeping my son/daughter at home if any symptoms of illness are present. *
I am comfortable sending my son/daughter to in-person conditioning practice when it is not required. *
I would feel more comfortable if my son/daughter could complete conditioning and practice at home when possible. *
