Souhlasím, aby mnou poskytnuté osobní údaje byly zpracovány a evidovány v databázi členů klubu AID Beroun za účelem registrace na semináři, evidence členské základny, stupňů technické vyspělosti a komunikaci se členy, dále zpracovávány pro účely orgánů státní správy a samosprávy v případě žádostí o dotace, a to v souladu s nařízením Evropského parlamentu a Rady (EU) 2016/679 o ochraně fyzických osob v souvislosti se zpracováním osobních údajů a o volném pohybu těchto údajů (GDPR).

I agree that the personal data provided by me will be processed and registered in the database of AID Beroun club members for the purpose of registration at the seminar, registration of membership, technical ranking and communication with members, further processed for state administration and self-government bodies in the case of grant applications in accordance with Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of individuals with regard to the processing of personal data and on the free movement of such data (GDPR).

Souhlasím s tím, že cvičím na vlastní nebezpečí, že si sám/sama sjednám úrazové nebo sportovní pojištění a dále že AID Beroun nebude hradit škodu, která vznikla v důsledku toho, že jsem porušil/a pravidla provozu výcviku nebo že jsem nedodržel/a pokyny vedoucího kurzu.

I agree that I train at my own risk, that I take out accident or sports insurance myself, and that AID Beroun will not cover the damage that has arisen as a result of violating the training rules or failing to follow the instructions of course leader.

Souhlasím s tím, že seminář bude zdokumentován pomocí fotografií a videa. Fotografie a videa následně budou vystaveny na webových stránkách z důvodu propagace AID Beroun.
