I want to connect with you! This form will be available Monday-Friday, 7 am-2:30 pm on days that school is open. Upon submitting a request, your Counselor will work to follow-up with you in the next few days, but will not be able to respond immediately.

This form is NOT to be filled out in the event of an emergency. If you are in need of immediate assistance, or are in a crisis situation while in the building, you need to seek out a nearby and/or trusted adult or go directly to the main office for immediate help.
