SPPS Athletic Registration Form (Fall 2020)
In order to complete registrations for fall sports, you must do the following:
1. Complete this google form
2. Have an up-to-date physical on file (good from the date of the physical for three years)
3. Complete the MSHSL Eligibility Statement and Annual Sports Health Questionnaire
4. Pay the registration fee prior to August 17, 2020 [$45 or $20 free-reduced lunch; football fee is $55].
