All high school students, grades 9-12, are invited to attend the annual MTHEA Teen Banquet at Woodmont Hills Church, 3710 Franklin Road, Nashville, TN. Families of our graduating seniors are also welcome to attend.

Complete this form after you purchase tickets for your party at mthea.org. Use this form to register an individual, couple, or your group of up to eight (8) people. Every effort will be made to seat the people on your registration form at one table.
