It's time to plan for the 2021-2022 school year, and in order to do so we need to know two pieces of information:

1. Whether your child(ren) will be returning to St. Rose St. Mary's School, and
2. Whether your child(ren) has a sibling that would like to attend St. Rose St. Mary's school for the 2021-2022 school year.

Please answer the following questions and submit this form by Friday, March 19, 2021.

After submitting the form, please send in your 2021-2022 Non-Refundable Registration Check to the School Office. Our registration fee is $50 per student and all checks should be made out to St. Rose St. Mary's School.
If we do not receive a check, your registration fee will be applied to your FACTs Account for 2021-2022.
*If you are a School Choice Family your fee is covered by your voucher.*

In August all returning families will receive an email with information on how to review and update PowerSchool information for the 2021-2022 school year.

**Families with students entering 4K, 5K, or 1st Grade don't forget to consider the School Choice Program!**
