Adults: Once this form is filled out, you are registered. However, you must be current (every 2 years) on background checks and Safeguarding God's Children trainings to volunteer. Please email youthfamily@salcvan.org if you are unsure of your status.
Youth: Once this form is filled out, an email with a medical form will be emailed to the email address provided. This must be returned to youthfamily@salcvan.org in order to complete your registration and to be a volunteer.
