If a contestant is entering both solo and duet/duo categories, please fill out a separate registration for each category.
Please do not submit another form if any errors are made or if you need to edit an entry. Instead, contact the competition chair.

Your information will only be seen by the chairperson of this competition and the CEOMTA treasurer (during payment process). It will only be used to contact the contestant/parent, for reasons including competition scheduling, registration ambiguities, or awards/certificates should the student win a prize in this year’s competition.
