Meal Request Form (Virtual & Non-School Age Students Only)
Based on an the extension of the summer meal waiver program by the USDA, we will be offering FREE meals (breakfast & lunch) to all families with children ages 1-18 beginning Tuesday, Sept. 8 and continuing through Dec. 18 (as federal funding allows).

Virtual families and families of all children, ages 1-18, in our community who want to take advantage of school breakfast/lunch should complete this form by Sunday, September 6th at 3:00 p.m.

Pick up will be in the front of the school building from 8:30-9:00 a.m. in the front entrance drive, and will include lunch, plus breakfast for the next day. If this time does not work for you, please contact Donna Hoover at dhoover@midwayk12.net.

We are excited to be able to provide FREE meals to ALL our Vikings!

Please be sure to fill out one (1) form for each student and complete it by 2:00 on Monday, September 7th.
