I hereby authorize the Camp Staff to act for me, according to their best judgement in any emergency requiring medical attention for my child, and waiver and release the camp from liability for any injury or illness incurred while at the camp.

I hereby consent to emergency medial or hospital service rendered to my child by or at accredited hospitals by appointed physicians in the event such need arises in the opinion of a duly licensed physician.
