Welcome to Middle School Extravaganza!!
Who: All rising middle schoolers are invited to celebrate our incoming 6th graders (and since this is our first one, all our incoming middle schoolers!)
What: Fort Fun, MOD Pizza, a cupcake decorating competition, all the best lock-in games, turning the Fellowship Hall into a Movie Theater, and so much more!!
When: May 27th, beginning at 3 p.m. and ending at 9 p.m.
Cost: $30 (Scholarships available!)

Any dietary restrictions?
My child has permission to attend and participate in the church trip activities, which may be held either on or off the church property.  Youth ministry staff and volunteers have my permission to drive my child during the event. I do hereby release, forever discharge and agree to hold harmless, the sponsoring churches and the directors thereof for any and all liability, claims, demands for personal injury or sickness, as well as property damage or loss of belongings and expenses, of any nature whatsoever which may be incurred by my child.  In the event medical attention is required, I understand that attempts will be made to inform me of the situation, however, I hereby give adult leaders, advisors, and/or volunteers authority to act in my stead in case of emergency and give medical staff permission to carry out any medical or surgical procedures as may be deemed necessary by any licensed medical staff.  I assume responsibility for all medical bills, if any. You must be at least 18 years old to sign.

I give my consent for photographs taken of the the participant during the event to be reproduced and/or used in promotional materials for the sponsoring churches.  I am aware that these photos will not be sold or used for profit other than for their presence in promotional materials, and I am aware that I will receive no compensation for the use of these photos.
