The purpose of this Evaluation (feedback) is to better understand your child's hockey experience and look for ways to improve. This evaluation form will assess the Head Coach (and at times bench members). Please remember to be fair and respectful when completing. The evaluation is meant to be corrective in nature, not punitive. Coaches will receive compiled feedback anonymously. If you wish to leave your name, you can do that in the 'Overall' comments section.
If possible, please have your player participate in the evaluation. One evaluation per player.
