Note: The barcode MUST remain on the calculator all year. If it becomes damaged, see Dr. Kennedy in the math office, room A238.
Note to Student: Please take the responsibility of borrowing a calculator seriously. In June, the calculator serial number MUST match the one issued to you. If it does not match, you are responsible for payment or replacement. Failure to make restitution will jeopardize your participation in your high school graduation ceremony. In the case of theft, please file a report with your assistant principal immediately.
I understand and acknowledge that the loaned calculator, signed out according to student ID and serial number, must be returned in working condition in June and that I will take responsibility for the replacement of the calculator if lost or stolen, damaged, or the serial number is removed or altered in any way. I also understand and acknowledge the replacement cost for the calculator is $119 for TI-84 model, and $129 for TI-89 model and batteries are the student's responsibility. Alternative to payment, students may purchase, and turn in to the school the same model calculator as a replacement (*purchased calculators with "school property" printed on them are NOT acceptable replacements). *
