This form is for the upcoming 2023-2024 school year.   You will need to provide your CMS ID, CPCC ID, and reliable student and parent email addresses/phone numbers so we can contact you.

As a reminder:  To be eligible to apply to the CMS lottery for any middle college, you must meet the eligibility requirements found here .
All steps of the application must be completed by 10:00 pm on the last day of the lottery.
