MiddleFest! 2023 Student Registration
Please fill out this form with your information for MiddleFest! 2023. Be very careful that you are accurate in your response. When you are finished, hit submit. Print the Financial Form provided and send your School Check or Money Order to the MiddleFest! Coordinator. ALL information must be entered by January 13, 2023.
