-Fill out this fillable Boat Slip lease permit

-Provide a valid boat registration

-Provide a valid boat insurance

-Provide an annual park pass or purchase one. The Utah State Park annual park pass is on a calendar year and will be good until December 31, 2023

-Complete the STD of the sea training and information  STDoftheSea.Utah.gov

I will provide these documents in person at the office and pay the appropriate fee or email them to kburden@utah.gov and then call the office to make payment over the phone
