Dear Ukiah Families,

We are excited to invite you to take the 2021-22 Ukiah Unified School District Family Survey. We need your feedback to make sure that we continue to provide high quality, rigorous learning experiences that meet the needs of our students. We want to know what you think.

Below is a survey for you to complete for each child who attends one of our schools. The results will be used to guide our work over the coming years. The survey will take approximately 10 minutes to complete.
