Safe Kids Strong Families Fun Run/Walk
Event Timing: Sunday, April 23, 2023, 8:30 AM check in (if you need a shirt), 9:00 AM start time
Event Location: Kemper Center, 6501 3rd Avenue, Kenosha, WI 53143

T-Shirt Pick Up: Wednesday, April 19, 11:00 - 1:00 pm or Thursday, April 20, 5:00 - 7:00 pm at the Kenosha County Job Center, 8600 Sheridan Road, Entrance B, or on the day of the event.

There will be no registration for t-shirts and they will be given out on a first come, first served basis.  Available sizes will be Youth XS- Adult 3X, with a limited quantity of each size.

Waiver of Liability.  This agreement releases agencies represented on the Safe Kids, Strong Families committee and the Kemper Center from all liability relating to injuries that may occur during the family fun run/walk at the Kemper Center on 4/23/23. I agree to hold agencies on the Safe Kids, Strong Families committee and the Kemper Center entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence.  I also acknowledge the risks involved in running/walking a fun run. These include but are not limited to breathing problems, broken bones, sprains, scrapes or blisters. I swear that I am participating voluntarily and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity.  I forfeit all right to bring a suit against agencies on the Safe Kids, Strong Families committee and the Kemper Center for any reason. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed.  I understand this waiver of liability also pertains to any minor children that are in my care during the duration of this event.  *

Consent for use of photographs. The Safe Kids, Strong Families committee has a Facebook page in which photographs and information are shared about the April events regarding Child Abuse and Neglect Prevention.  Photographs will be taken during the Family Fun Run/Walk on 4/23/23 at Kemper Center.  I give permission to the Safe Kids, Strong Families Campaign to use photographs of myself and/or my family, including children, in the event they would do a follow up Facebook posting or a news article for publication. I understand that if I do not agree to the publication of photos, it is my responsibility to contact a volunteer at the event so they are aware of who I am.   *
