If you fill out this form, you do NOT need to fill out our Parking Agreement form. Your vehicle will be parked at Ceiba during your trip, and the parking fee will be incorporated in to your invoice. When filling this out, you can use the Back button to change any responses before submitting. After submitting, if you plan to pay for the shuttle yourself on a separate personal invoice, contact our office to confirm and pay.
We will not shuttle a vehicle until this form is completed.
After you submit this form, you will receive an email from ceibagc@gmail.com with your completed form. Please open and review it to make sure everything is correct. If it all looks good, print out the completed form and stow it somewhere in your vehicle. You can then refer to it when you park your vehicle for the trip and are wondering what you originally said on the form. We want to make sure we are on the same page with where you say your keys/gas money are actually hidden.
