Covid-19, Quarantines, and Incarceration: Exploring the Resiliency of Individuals and Families Who Have Experienced Incarceration
Informed Consent

INTRODUCTION:
For this study, we are exploring how people are experiencing various levels of quarantine. Specifically we are wondering if individuals who have experienced incarceration, either personally or through the incarceration of a loved one, have learned tools that have built their resiliency. Are these individuals feeling less lonely during periods of quarantine related to Covid-19? And we are wondering what some of these tools are that are helping them more successfully navigate these experiences.

DESCRIPTION OF RESEARCH STUDY:
If you decide to participate, you will complete an online survey. The survey should take you about 45 minutes to complete. There will be questions about your experiences with Covid-19 quarantine, Loneliness, how you’re coping with the quarantine, etc. For those who have experienced incarceration, there will be questions about your length of incarceration. Finally, for those who experienced the incarceration of a loved one, we will ask about those relationships and how you were able to maintain those relationships, during periods of incarceration. At the end of the survey, you will be asked if you would like to be informed of the results. Dr. Lazzari is the only person who will have access to personal information collected in the survey. You will hear directly from her, if you are interested in learning about the results.

RISKS AND BENEFITS:

RISKS:
There is always an element of risk present, when participating in research studies. However, we will do our best to maintain your confidentiality. If you decide to participate in this study, and if you are asked about personal experiences and/or opinions-- then you face a risk of identifying negative thoughts or feelings related to unpleasant past/present experiences, so there is a remote chance that answering these questions might make you feel anxious or ill at ease. If this occurs, you may stop from participating at any time without penalty. If  during the course of the survey, you are in need of any counseling services, please visit www.psychologytoday.com/us/therapists. They offer counseling, teletherapy, support groups, and psychiatric services. Educational information can also be provided upon request. You can always reach out to Dr. Lazzari, if you have questions.

Similarly, since the survey is online, there is an element of risk associated with sending emails and downloading materials. However, your personal information will not be shared with anyone, other than Dr. Lazzari, and your responses will not be able to be tied to your identity. The researchers will try to reduce these risks by assuring the confidentiality of your responses, and by identifying any support services available in your area if needed, or by the ability to discontinue at any time. However, there is some possibility that you may be subject to risks that have not yet been identified, due to the potentially personal nature of the questions.

BENEFITS:
The main benefit for participating in this study is to increase our understanding of how various groups of people are being impacted by Covid-19. We also want to begin exploring the resiliency of those who have experienced some form of incarceration.

COSTS AND PAYMENTS:
The researchers want your decision about participating in this study to be absolutely voluntary. The researchers are unable to give you any payment for participating in this study. We will, however, provide access to the outcomes of the study, and we will share any resulting publications related to the study.

NEW INFORMATION:
If the researchers find new information during this study that would reasonably change your decision about participating, then they will give it to you.

CONFIDENTIALITY:
The researchers will take “reasonable” steps to keep private information. Only Dr. Lazzari will have access to all of the responses on the survey. Dr. Lazzari will share pieces of the responses with the research team. But any personal information will be kept locked and secured by Dr. Lazzari. The results in this study may be used in reports, presentations, and publications; but the researchers will not identify you. Confidentiality will be maintained to the limits of the law. Confidentiality may not be maintained if you indicate that you may do harm to yourself or are placing yourself in an immediate harmful situation-or if you may do/have done harm to others. In cases where one is thought to pose an imminent risk of harm, the appropriate health care providers and/or parents/guardians may be contacted.

All responses collected in this research will be analyzed in aggregate form, and no names will be linked to the analysis. Consent forms are separated and not linked to identifying information and they will be kept for a period of 3 years in the secure place (Heidelberg University, indicate specific location).
Your confidentiality will also be maintained to the degree permitted by the technology used, thus no guarantees can be made regarding the interception of data sent via the Internet, web-based services and/or by any third parties, but every attempt to safeguard your data will be made.

WITHDRAWAL PRIVILEGE:
Participation is voluntary and you may withdraw at any time. You will not be penalized or forfeit any legal rights for choosing to withdraw. Your information will remain confidential and your name will not appear on any study materials.

It is OK for you to say NO.  Even if you say YES now, you are free to say NO later, and walk away or withdraw from the study at any time.  Your decision will not affect your relationship with Heidelberg University, or otherwise cause a loss of benefits to which you might otherwise be entitled.  Taking part in this project is entirely up to you, and no one will hold it against you if you decide not to do it.  If you do take part, you may stop at any time.

1. You are saying that you have read this form or have had it read to you, that you are satisfied that you understand this form, the research study, and its risks and benefits.
2. You are at least 18 years of age, and wish to participate in the study.
3. You are not currently incarcerated.
4. You understand that if you have any questions later on, you can always reach out to Dr. Lazzari. Her email is at the top and the bottom of this form.
5. You wish to participate, but you can stop at any point.
