The information you provide will be used for several purposes.  Specifically, the Nova Scotia Department of Education requires us to provide your planned destination after high school.  

Also, the information you provide will be used during the grade 12 prize meetings and scholarship/bursary information is shared during the graduation ceremony.

If you do not submit the form by this date, you will not be considered for the prizes available to graduating students.
