The safety of your child is our primary concern. All precautions will be taken for their well-being and protection.

I/we, the parents or guardians named above, authorize representatives of Hamilton Chinese Alliance Church and Hamilton Grace Mandarin Alliance Church to sign a consent for emergency medical treatment and to authorize any physician or hospital to provide medical assess- ment, treatment or procedures for the child named above, as deemed necessary.

I/we, named above, undertake and agree to indemnify and hold blameless Hamilton Chinese Alliance Church and Hamilton Grace Mandarin Alliance Church, its Pastors, Board of Elders and Group leaders from and against any loss, damage or injury suffered by the participant as a result of being part of the church’s Children Ministry, as well as of any medical treatment authorized by the super- vising individuals representing the church.

I/we, named above, undertake and agree to ensure my child attends VBS during the week only if they are not experiencing any COVID symptoms. If they are, I agree to not allow my child to attend VBS to ensure the safety of VBS’ campers and volunteers.

I/we have read, understood and agreed to the above and hereby give permission for my child named above to partici- pate in Children’s Ministry activities at Hamilton Chinese Alliance Church for the period of Vacation Bible School from August 8th to August 12th.
