Parents, completing this form will provide our student ministry volunteers with valuable information about your student that we hope will empower him/her to thrive in our ministries. We understand that every student is different and that some students have particular needs that won't be met within the usual parameters of our ministry. However, we desire to faithfully and effectively serve every family/student that attends our ministries, and this form will help us to serve your student. Please complete all relevant portions of the form
