In consideration of being permitted to participate in the 2019 Exilic Dodgeball Tournament, scheduled for December 7, 2019, I acknowledge the possibility of injuries, damages, or losses resulting from participation in this event, and I agree to assume all risks of any such injuries, damages, or losses, regardless of their severity, that my minor child/ward or I may sustain as a result of participating in the activities that are in any way connected to or associated with this event.

I agree to release and forever discharge Exilic staff, their administrators, Fashion High School, and all other involved parties from any and all claims of injuries (even injuries resulting in death of the participant), damages, or losses that my minor child/ward or I may sustain as a result of participating in the activities that are in any way connected to or associated with this event.

I acknowledge and agree that I have read and fully understood this Waiver of Liability agreement, that I have read and explained the same to my minor child/ward, and that I am freely and voluntarily executing the same.
