THIS FORM IS FOR PARENT/LEGAL GUARDIANS OF STUDENTS CURRENTLY ENROLLED IN A SCCS SCHOOL.  Please fill out and submit this form to request an authorization code to access the Illuminate Parent Portal.  If you have more than one student in the district during the 2020-21 school year, please fill out the form for only one; the current siblings, and siblings enrolling for the 2020-21 school year, will be automatically connected by the system.
