Each parent/guardian must fill out this form each morning before 8:15 a.m. for each individual remote learner. If there are two students in a household, the parent/guardian should fill out this GoogleForm twice each day (1 GoogleForm for student A and a second GoogleForm for student B).

Answer each question below. This Google Form will be used as attendance of the school each morning.
