BRITISH CYCLING CONSENT
DECLARATION In the case of the applicant being over 18 years of age, the following must be read and signed: I declare that the above information is correct and that the person in charge has my permission to authorise medical treatment in an emergency. If I have answered “yes” to one or more of the above questions in Section 4, I should seek medical advice before attending a ride. I agree to tell the mountain bike leader if there is a change in my medical condition. I understand that this information will be shared with other mountain bike leaders and that I cycle at my own risk. I ACCEPT THAT THERE IS AN INHERENT RISK OF INJURY IN PARTICIPATION IN CYCLING ACTIVITIES. RISK CAN BE REDUCED TO ACCEPTABLE LEVELS BY IMPLEMENTING APPROPRIATE RISK ASSESSMENTS. COPIES OF WRITTEN RISK ASSESSMENTS ARE AVAILABLE ON REQUEST.
The ride is insured in respect of legal liabilities (third party liability). However, participants have no personal accident cover unless they have been specifically advised of this in writing by the organiser of the ride. It is the participants’ responsibility to arrange for any extension of insurance cover unless advised differently by the leader or the organiser of the ride.
DATA PROTECTION STATEMENT British Cycling respects individual privacy and has notified (registered with) the Information Commissioner, who is responsible for the administration of the Data Protection Act 1998 (the Act). British Cycling obtains and processes “personal data” (as defined by the Act) for the purpose of administering its functions as a National Sporting Governing Body and under relevant legislation. In addition, British Cycling may use information for the purpose of fulfilling its statutory responsibilities under the Data Protection Act 1998. Information will be kept secure and confidential and will only be disclosed to those parties who have a legal and legitimate need to know.

I am aware that cycling is an inherently risky activity that involves the possibility of injury or death. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, lack of hydration, and actions of other people including, but not limited to, participants, volunteers, monitors, and/or producers of the activity.
I have read the event joining information and understand that all applicable guidelines for participation must be followed and that sole responsibility for my personal safety lies with me, including my physical and mental preparation and fitness to participate in the event
I am aware that this event is being held on open public roads, bridleways, and other shared paths, and the Organisers cannot be held responsible for actions, accidents or incidents caused by other road or trail users.
I undertake and agree to remove myself from participation if I sense or observe any unusual hazard or unsafe condition, or if, during the event I feel unable or unfit to safely continue for any reason
I agree that the event organisers nor their partners or agents can accept no liability for any theft or loss of personal possessions during the event, including but not limited to bike. Participants are responsible for their possessions and any insurance they wish to carry
It is my responsibility to make a decision whether to ride a section or not. The route in the directions does not necessarily indicate that a section is rideable and the participant is deemed to make their own decision about whether to proceed on bike, foot or not at all. Any route provided is for guidance only and Lakes Gravel Gang does not insist that you use the route shown. If I choose a different route to avoid obstacles or sections I do so entirely at my own risk.
If, during the event, I decide to change the route and distance, consequences of the actions will be mine.
I understand that the rules of the Highway Code must be followed at all times, and I am aware that trail and road awareness and safety of myself and other trail and road users must be my primary concern
I’m aware this is not a race in any format
I confirm that the bicycle that I am riding and all of the equipment is of a suitable standard for the terrain and in a suitable state of repair, and that I am comfortable riding it on the terrain detailed in the joining instructions.
I agree to wear, as a minimum, a suitable designed cycling helmet during the whole riding section of the event. I agree to carry the recommended kit provided in the joining instructions, including hydrations, food, tools, medical and safety equipment.
I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity. I agree to carry medical supplies required for treating any medical conditions specific to me.
I agree to sign out and back in of all rides so event organisers are aware of when I have left and returned safely from the ride.
I agree to inform event organisers if there is an incident, and complete an incident report.
I am responsible for providing, if desired, my own insurance protection for my participation in the event. Any accident or injury caused by the rider's actions will be the fully responsibility of the said rider.
I acknowledge that images may be captured during the event using film photography, digital photography, video or other media and may be used on social media websites, promotional leaflets, newsletters and other publicity materials. The images may also be provided to the media for publication in local or national media
I agree not to attend the event if I am infected by COVID
I understand it is my responsibility to ensure I am physically and mentally fit to take part in this event , if I suffer from any medical condition or illness that may affect my fitness or ability to take part in this event it is my personal choice to take part and the Organisers accept NO LIABILITY for exacerbating my condition or any ill affects I may suffer during or after the event.
I give, a full release and waiver of any liability and all claims that I have, or may have in the future, against The Organisers, volunteers, agents, vendors, partners, or contractors to the event. from all liability for any loss or expense, damage, injury, death that I may suffer as a result of my participation in any part or parts of this event or my presence at any venue at which they may take place, due to any cause whatsoever
