Dear Parents/Carers,

In order to help with our preparations for September, we would appreciate if you could complete the form below regarding milk for the autumn term.

The cost of mid-morning milk for pupils is £12.00 for the term. This is a standard charge per term and will cover the period September to December 2020. Refunds will not be given due to the absence of children from school or any INSET days taken by the school.

If you would like your child to have milk in the autumn term, please complete the form below and you can pay at the school office.

All pupils in Reception and Nursery will automatically receive milk free of charge due to their age.

The deadline for Milk payment/orders is Monday 27th July. After this date our order will be placed with the dairy and cannot be amended.
