This form must be completed to inform Kiwi Cabins Custom Builds Ltd of your authorisation regarding onsite delivery at New Zealand's Covid-19 Alert Level 2.

I understand and agree to what is involved regarding the Kiwi Cabins team delivering & assembling on my site. *

I will maintain social distancing of 2m minimum from the team when they are onsite delivering and assembling my product. *

I will understand these precautions are in place for the safety of the Kiwi Cabins staff and myself. *

I have the authorisation to allow Kiwi Cabins on the delivery property in question during Alert Level 2. *

I agree that Kiwi Cabins may deliver and assemble on my property during Alert Level 2. *
