Membership Application Form for BayLUG & BayLTC

We have three membership types:

1. Individual Adult = one adult (18+) member
2. Individual Minor = one minor (under 18) member - must be supervised by an adult parent/guardian at club events (who does not need to be a member).
3. Family = up to 6 members in one household including at least one adult

Family dues are 2x the Individual dues ($40/year for family and $20/year for Individual).

For Individual members, enter your last name. For Family members, enter the last name of the primary contact person (must be an adult).

Year of Birth (YYYY) *

Badge Name *

Your name as you would like to appear on your club badge. Use just your first name, your full name, or a nickname. For Family members this is the badge of the primary contact adult.

Never submit passwords through Google Forms.
