SHA Withdrawal Form

Please use this form to communicate your intention to withdraw from SHA. Please note that this is a first step in this process and we require confirmation enrollment information from the new school, all SPS and SHA properties to be confirmed as returned, before the withdrawal is complete. The timeline for this process may be impacted by school building closures.
