2023-2024 USAC Judge Evaluation Form
This form was created to collect references for Boulder and Lead/TR Climbing Judges at the USA Climbing competitions in the 2023-2024 season.  This form is designed for reviewers with Level 2 or greater certification.

The Judges Committee would greatly appreciate you taking a few minutes to fill out the requested information below for each route or boulder judge whom you worked with at Youth Championship events, National events, and IFSC events. This form is not for collecting references for Qualification Events.

Information provided here will be used to help the Judges Committee evaluate requests for higher-level judge certification.  Information provided here will be kept confidential (within the Judges Committee and USAC).
