1. I agree to the recording & utilization of the real-time interactive video for the lectures on the day of When two become one

2. I agree that I will not record & distribute real-time interactive training for lectures on the day of When two become one
「1st Couple's Day」held at  Gangdong-gu Health Family Multicultural Family Center and I will not raise any objection to illegal recording, screening or distribution to others in the future.

3. I agree to the collection & Use of personal information as follows in accordance with relevant laws such as Article 15 Paragraph 1 of the Personal Information Protection Act.
1) Items of Personal Information to be collected : Name, Birthdate, Contact Information.
2) Purpose Collection & Use of Personal Information : Confirmation of consent for real-time video recording.
3) Personal Information Retention & Period of Use : Automatic destruction of the flat-form logout upon completion of training for use & retention period.

4.  I am applying for participation in the 8th All in one Online Family Festival [Family Cheerful Music Festival] hosted by the Gangdong-gu Healthy Family Multicultural Family Support Center. All the photos and videos taken in this contest are Center's All rights, including copyright and portrait rights.

5. I acknowledge and agree that the restrictions may apply in Gangdong-gu Healthy Family Multicultural Family Support Center conducting the 8th All-in- one Online Family Festival [Family Cheerful Music Festival] if I do not attend the program after acquiring the materials/kits distributed or do not carry out activities.

  - Document Management : Stored during the service support period & destroyed after document storage institution specified in
    the personal Information processing policy after service information.

4) Right to Refuse Consent
  - You may refuse to consent to the collection & use of personal information above. However, if you refuse to consent you may
    be denied access to the Gangdong-gu Health Family Multicultural Family Center's service.

  - Local Government ( Seoul, Gangdong-gu ) provided to organization such as Ministry of Gender and Equality and Family, Korea
    Health and Family Promotion Agency and other Health Family Multicultural Family Support Center.
2) Purpose of Use of the Recipient
  - Personal Identification & Service History Management
  - Telephone satisfaction survey according to use of service.
  - Health Family Center Management  ( Performance Management & Promotion, Jurisdiction Center Support).
3) Provided Items ( Name, Birthdate, Address, Contact , Email, Job, Photo, Companion Participation Family Info, Satisfaction etc.
4) Retention & Use Period of the Recipient
  - Document Management : Stored during the service support period and destroyed after the documents retention period
    specified in the policy after the service ends.
