Approximately how many first responders do you expect will attend the workshop?

*

Check all resources which will be available for the classroom portion:

*
Required

Are you able to print the handouts (13-15 pages) double-sided in color? [Note: PDF can be emailed to participants]

If the prefered location is outdoors, what is the backup plan in case of inclement weather? In the example below, it was a loading dock area at the host site. A covered parking area or inside a firestation works well.

*

Approximately how many cars do you think will fit in the designated parking space for the hands-on demo, leaving sufficient room between cars to walk and open doors?

*

Are you able to host the workshop on a weekend day?

*

If you answered no or maybe to previous question, what is your preferred day of week? Weeknights are evaluated on a case-by-case basis depending on the distance the instructors need to travel.

If held on a Saturday or Sunday, what 4-hour time block do you prefer? Includes a 30 min to 1 hr bathroom, snack, or lunch break.

Clear selection

Do you have a date, month, or season in mind when you would like to hold the workshop in the next 12 months? If the date(s) you suggest are not available, we will respond with some alternate dates.

*

Do you have any questions or specific requests? Once the workshop is scheduled, we will provide a draft flyer you can modify to advertise the workshop.
