I declare that I am the legal parent, caregiver and/or guardian (the Parent) of the nominated player (the Player) above. I agree in full to the following terms and conditions as part of my registration for the Nerang Soccer Club & Academy of Football Australia off season training trials and talent ID soccer sessions and program. I agree —

• To absolve, release from and indemnify Nerang Soccer Club Inc and Academy of Football Australia, including any coach, staff member, official, volunteer or administrator (The Club) from any liability or responsibility for any injury, loss, damages or impacts that may be incurred by participating in the HPM Trials Program and Session (the Program), to the full extent by which the law permits.
• To not bring about any claim for damages or seek any compensation, renumeration or payment from the The Club or any of its partners or member organisations, criminal or civil, for any reason or purpose, within the full extent by which the law permits.
• To conduct myself and supervise my child to conduct themselves at all times to meet the standards set forth by the Football Federation Australia, Football Queensland, Football Gold Coast and Nerang Soccer Club – including but not limited to expected codes of behaviour, fair play and respect for all parties involved.
• To accept that the AFA and NSC programs are advanced training programs that are designed for aspiring intermediate and advanced junior soccer players – it is not a beginner program or for basic football training.
• I accept that soccer (football) game-play and training is a physical contact activity and authorise the coaches, staff and/or The Club's officials to administer any reasonable or required first response and first aid attention to any player deemed injured or in need of first aid or medical assistance or care, and that they may also call for an ambulance or further medical attention if deemed necessary by The Club's officials and I accept that any costs or liabilities arising from such events, injuries, treatments, first aid and recovery are entirely not the liability of The Clubs (Nerang Soccer Club and Academy of Football Australia), their coaches, staff, officials or any other participants in this program.
• I accept and agree that myself and my child will participate in the program by observing and following all instructions and directions from club officials, coaches and club staff at all times and that I and my child must adhere at all times to all Football Federation Australia, Football Queensland, Football Gold Coast and Nerang Soccer Club rules and regulations, including those that relate specifically to participation, behaviour and conduct, and that failure to do so may result in removal of myself and my child from the HPM program at the sole discretion of the Nerang Soccer Club officials.
• I agree that this program is for intermediate and advanced players and accept in full that acceptance into and participation in the program is at the discretion of coaches and officials and The Club, and that a refund may be issued and players refused participation at the sole discretion of The Club and Nerang Soccer Club committee.
• I accept and agree that swearing, bullying, teasing, violence, racism, hate speech, deliberate acts of harm and anti-social behaviour is strictly prohibited in this program and players or parents who exhibit such behaviour may be removed from the program at the sole discretion of Nerang Soccer Club officials.
• I agree to promptly pay the nominated fee for my player's age bracket and that non-payment may result in the immediate removal of The Player from the program at the discretion of The Club.

PAYMENT DETAILS PROVIDED AFTER FORM IS SUBMITTED
