Mental Health Screening Form for Graduate Students
Mental health screening form is an initial mental health assessment tool and a screening for potential mental health problems. The form is conducted to identify people at risk for mental health problems and consists of three parts: general information, a 2-question depression screening questionnaire, and a 9-question screening questionnaire.

If the screening results indicate that you are at risk for mental health problems, you will be asked about receiving counseling service. If you want to receive counseling service, the Office of the Postgraduate Studies will contact you again for more information. If you do not want the counselling service, the Office will send information on how to access the service for your further consideration. And all information collected will be kept confidential and will be used for further evaluation and the University's mental health monitoring.
