Please use this form to report concerns about a student's ability to appropriately access their education due to the actions of another member of our community.  Note that an aggressor or perpetrator can be any member of our school community, including a staff member.

This form can also be used by SPS staff members who wish to make a formal report of discrimination, harrassament or civil rights violations that involves a student as a target.

The information you provide will be emailed to a building administrator within the school that you select below.  These staff members will check their email DURING REGULAR SCHOOL HOURS.

NOTE: You can make this report completely anonymous by not providing us with your name (we won't know who you are); however, it may impede the investigation in the event that we have clarifying questions about your report.  If you choose to tell us who you are, we can maintain your anonymity in most cases.

Also, in most cases, traditional disciplinary action cannot be taken against an alleged aggressor solely on the basis of an anonymous report.
